Start with the work that is getting stuck
An existing site and department, a named location, reasons for raising a signal, and the people who should be notified.
Keep the next step visible
Set up and print a QR sign. Anyone at the location can select a reason and optionally add details, a photo, or a name. Staff acknowledge and resolve the signal, discuss it in the Daily Huddle, and create or link an improvement when further work is needed.
See what moved and what needs attention
